How do I request a home visit?

Home visits take considerably more time for GPs and Nurses than seeing patients at the surgery. Therefore, we encourage patients to come to the surgeries whenever possible.

To request a home visit, please contact the surgery before 11 o’clock. Our receptionists will need to know some details in order to give the GPs an idea of the priority for a visit. Please note that even by taking your details, the Receptionists are not guaranteeing a visit. A member of the clinical team may telephone you back for more information, and it is possible you will be invited into the surgery for an appointment, if a visit is felt to be unnecessary. A lack of easily available transport is not necessarily a reason to need a home visit.

We find it very irritating to be asked to see Patients at home, to find that they are out. If you can get to the shops, with your family, or to the hairdressers, generally you can also get to the surgery.

PLEASE COME IN WHENEVER POSSIBLE.
    
    

How do I speak to a Doctor or Nurse?


The GPs and Nurses are very happy to talk to patients if required. When you telephone the surgery, please ask the Receptionist if you can speak to the GP or Nurse. If the person is unavailable, you may be asked to call back later, or the Receptionist may take your telephone number.

We regret that GPs and Nurses will not be disturbed during the consultations unless it is a life-threatening emergency.

Each day, there is a GP on-call to deal with urgent problems, who can talk to you, if you feel your problem does not need a visit to the surgery.
    
    

How do I get medical help after the surgery has closed?

To seek medical help after the surgery has closed, please telephone 08450 568 060, and a Receptionist will take you details to pass them on to the Doctor on call, who will telephone you back.

Many problems can be resolved by phone, but you may be invited to attend the Unscheduled Care Centre, if it is felt necessary. Home visits are discouraged, and a lack of easily available transport is not a reason to have a home visit.
    
    

How do I order a repeat prescription?

If your doctor has agreed, you may obtain medicines on a repeat prescription without having to see him/her. However, you may be asked to attend a review when your condition needs checking.

You may get a repeat prescription by the following methods :

·    bringing in your slip, fully completed with your requirements, to the prescription desk

·    posting it in the letter box if we are closed.

·    By email on: 'Repeat Prescriptions ' or by typing repeats@posterngate.co.uk in the 'To' line on you email sender.


Please note that at present we do not offer an encryption service for e-mail requests, and therefore this information is passed at your own risk. If you have any concerns about this, please use an alternative method to order your repeat prescription.

We require one working day's notice to ensure prescriptions are checked and signed, and ask that you wait until after 2pm to collect your prescription.

How do I request a sick note?


GPs do not issue sick notes until you have been off work for 7 days. Before this you need to complete a self certification form.

To obtain a sick note, you need to be seen by the GP. Please note that backdating of new notes is not possible under any circumstances.

To obtain a continuing sick note, provided the GP is agreeable, you may telephone requesting one. If the GP is in any doubt, you will be asked to attend the surgery.

If a patient has been in hospital, the hospital should provide a note to cover that time. GPs are allowed to issue sick notes, if they have received a letter form another Doctor, which makes it clear why a sick note is required.

How do I get a blood test taken?

If a blood test is appropriate the practice will issue a blood test request form.

We encourage patients to attend the Selby Hospital service as much as possible.

These are held at Selby War Memorial Hospital, outpatient department, and at York Hospital Trust Phlebotomy clinic, York Hospital, Wigginton Road, York, YO31 8HE.

YOU WILL NEED TO MAKE AN APPOINTMENT FOR A BLOOD TEST at the hospital, please call Selby Hospital on 01904 724310 between 9am-12 midday. The line is usually quieter after 1030. Appointments can be made up to 2 weeks in advance.

The clinic operates Mon-Fri 8.30 am - 12.15pm & Mon afternoon 1pm-4.20pm. Five minutes is allocated for each appointment. 

Please note, 'drop in' blood taking sessions are held at York Hospital during normal office hours and at ASDA Monks Cross Monday - Thursday 8.30am - 4.30pm and Friday 8.30 am - 2.45pm. 

REMEMBER TO TAKE YOUR BLOOD FORM WITH YOU TO THESE APPOINTMENTS, AS BLOOD SAMPLES CANNOT BE TAKEN WITHOUT COMPLETED FORMS.  


Due to the travelling involved, we are able to offer a service at branch surgeries on a weekly basis. Please ask at Reception about using this service.

Exceptionally, if patients are housebound, we can arrange for a Nurse to come to the house.
    
    

How do I get my test results?

Most blood test results are returned to the surgery within 48 hours, some often sooner than this. You can telephone the surgery after 2pm for results of blood tests, urine specimens and other swabs.

We regret that confidentiality requirements do not permit us to disclose results to a third party, even if it is a spouse. If the member of staff is in any doubt as to whom they are speaking, they are authorised to refuse the request.

Cervical smear results are automatically sent to the patient (unless a specific request has been made for this to be avoided).    
    

How do I arrange a Medical Examination?

We undertake Medical examinations for people, but before booking an appintment we require a non-returnable deposit of £50.00.

Appointments need to be arranged separately, via the Reception team. Please note that we offer these examinations outside our commitments to the NHS, and therefore are limited in the times we are prepared to offer.
    
    

How do I obtain Emergency Contraception?

Emergency contraception is available from a number of sources.

* From the surgery, from either the GPs or the Nurses.

* From Minor Injuries at Selby War Memorial Hospital 24 hours a day.

* From Family Planning Clinics.

* From certain Pharmacies - over the counter, sometimes this will involve a fee. 
    

How do I register as a Carer?

If you are a carer (look after someone) please let us know by either informing the reception staff or GP so that we can enter it onto our computer records. This will make us aware that you may need extra consideration regarding making an appointment or a visit.
